Required Qualifications/Skills/Experience

• At least 4 years of experience as a clinical MTM pharmacist (management experience preferred)

• Familiarity with driving Part D Star measures, specifically SUPD, SPC, and CMR

• Understanding of pharmaceutical and medical vocabulary

• Clinical experience in medication review

• Fluency in speaking, reading, and writing English

• Excellent verbal and written communication skills

• Tech-savvy

Preferred Qualifications/Skills/Experience

• Experience using Salesforce

Pharmacist – MTM Overview

• The pharmacist will play a key role in delivering medication therapy management (MTM) services.

• This includes conducting outbound calls to complete comprehensive medication reviews (CMRs) and providing thorough consultations, particularly around statin use aligned with AHA guidelines.

• The pharmacist will transcribe and input patient-specific prescription, diagnosis, and recommendation data into a third-party, web-based platform.

• This information, drawn from either hard-copy sources or internal applications, is used to generate standardized documentation mailed to patients.

• The pharmacist will collaborate as part of the MTM product team and is expected to demonstrate professionalism, efficiency, and a patient-focused approach.

Job Duties

• Conduct outbound phone calls (approximately 100 calls per day)

• Complete comprehensive medication reviews (CMRs)

• Deliver medication therapy management services

• Accurately and efficiently transcribe patient-specific data into a third-party, web-based system

• Maintain confidentiality and adhere to HIPAA and privacy standards

• A valid Florida Pharmacist License is required

• Candidates must have MTM experience and familiarity with SUPD, SPC, and CMR measures
